Les parents de ce pre-adolescent de quatorze ans appelle le neurologue en raison d'une recrudescence récente des crises épileptiques de leur fils.

Sa pathologie épileptique est due à une malformation du cortex cérébral ; qui a aussi pour conséquence un retard sévère du développement.

 

Le neurologue, qui suit cet enfant depuis de nombreuses années, a modifié à plusieurs reprises la thérapeutique anticomitiale, obtenant un certain équilibre avec une bi-thérapie : Dépakine à dose efficace et Rivotril (benzodiazépine) à faible dose. 

Les essais d'augmentation de la thérapeutique n'ont pas permis un meilleur résultat clinique. 

Cet enfant présente par périodes des absences complexes, et annuellement deux à trois grandes crises. Il peut rester quelques semaines sans faire aucune crise. 

Depuis deux à trois semaines, ce jeune fait plusieurs crises « fortes » et répétées ; plusieurs par jour.

La question des parents porte sur l'attitude thérapeutique à adopter.
